I _________________________________ do hereby entrust Affordable Grooming and Boarding, Inc. to care for _____________________ for their boarding stay from _____________ to _______________ and agree to all of the terms listed below.

I authorize Affordable Grooming and Boarding, Inc. to do whatever they deem necessary for the health and well being of ____________________ and agree to pay for any and all expenses relating to same.

I agree to pay daily boarding charges at Affordable Grooming and Boarding, Inc.'s current posted rates. If my pet requires special care (i.e. emergency medical treatments, etc.) I authorize Affordable Grooming and Boarding, Inc. to provide such care and I agree to pay for those services.  I am aware that I will be solely responsible for any/all charges, anticipated or unanticipated, as well as for any costs that are incurred in the event of injury or illness.

I agree to pay for any auxiliary services requested by me (i.e. playtime, transportation, grooming, medicating, etc.). ** Please note: dispensing of medication will be charges at a rate of $1.00 per prescribed dosage.

I agree that I am solely responsible for any damages that _____________________ may cause through malicious or improper conduct.  Affordable Grooming and Boarding, Inc. will not be responsible for ___________________'s personal belongings that may be lost or damaged. Any belongings left at Affordable Grooming and Boarding, Inc. will become the property of affordable Grooming and Boarding, Inc. All blankets and toys left with ___________________ at check-in will be considered a kennel donation.

I understand that boarding charges begin on the day that _____________________ is dropped off and will not terminate until the scheduled departure date listed above.  If I decide to pick up __________________ earlier than the contracted pickup date, I understand that I will still be charged for the full contracted stay since he kennel space what been pre-booked for ___________________.  I agree to notify Affordable Grooming and Boarding, Inc. prior to the drop off day of any changes in __________________'s original reservation.

I understand that ________________________ must be picked up by 10:00am on their scheduled departure date to avoid being charged an additional full day's boarding rate.  I also understand that my pet will not be released to me until all charges are paid in full.

I understand that any pet left uncalled for 10 days after their scheduled pick-up date will become the property of Affordable Grooming and Boarding, Inc. and that I will be responsible for any costs incurred by Affordable Grooming and Boarding, Inc. while my pet remains under their supervision.

I understand that I will be responsible for any charges incurred by Affordable Grooming and Boarding, Inc. relating to any attorney fees and/or the collection of any unpaid charges that I may owe to Affordable Grooming and Boarding, Inc. for services rendered.

I understand that, should _________________________ escape, become ill, be injured or expire while under Affordable Grooming and Boarding, Inc.'s care, owners Al and Kerry Jones or employees of Affordable Grooming and Boarding, Inc. will not be held liable in any way.

I understand that boarding rates will change during Affordable Grooming and Boarding, Inc.'s peak season.
