TRAINING FACILITY - RENTAL RELEASE FORM

RULES AND REGULATIONS

• I agree to use the training facility at the current rate of $25.00 per hour.

• I am responsible for picking up after myself and my pet and for leaving the training facility clean, the gate closed, and the area in satisfactory condition for the next training session.

• I am responsible for returning things to the proper place if I move any equipment.

• I will advise Affordable Grooming and Boarding, Inc. immediately if I have any concerns with the training facility or if any equipment may become broken, or in some way, damaged, unusable, or unsafe.

• I will not allow any other persons and/or animals to use the facility while I am renting the training field UNLESS I have received permission to do so.

I have read this agreement and fully understand it's contents and accept it's terms and conditions.  I realize that, if I violate any of the above notes, rules and regulations, I will be prohibited from renting and/or using the training facility for future use.

I hereby release AFFORDABLE GROOMING & BOARDING, Inc., it's owners, Al & Kerry Jones and it's employees from any liability or responsibility for injuries and/or damages to either myself or my dog due to my use of, or participation in, any activities while renting/using the Training Facility.
